2025 Less than minimum number application form

This form is to be used for Clubs who wish to apply to perform with less than minumum numbers at competitions in 2025.

Please note, you can only apply for minimum of 4 to perform. Applications for 3 performers will not be approved.

Where possible, this form must be submitted a minimum of 2 weeks prior to the competition.
